How Old Toongabbie Pharmacy handles personal and health information.

Who we are
Old Toongabbie Pharmacy, Shop 2, 58–62 Fitzwilliam Road, Old Toongabbie NSW 2146, is a community pharmacy and a member of the MediADVICE Pharmacy network. This policy explains how we collect, use, disclose and protect personal information, including health information.
What we collect
- Contact details — name, address, phone number, email address, date of birth.
- Health information — prescriptions dispensed, medicines supplied, allergies and adverse reactions, immunisation history, and information you give us during a consultation or health check.
- Identifiers — Medicare, concession, DVA, Safety Net and Individual Healthcare Identifier numbers where these apply.
- Transaction records — what you purchased and how you paid.
Why we collect it
To dispense your medicines safely and lawfully, to provide the professional services you ask us for, to claim under the Pharmaceutical Benefits Scheme and other government programs, to contact you about your medicines or an appointment, and to meet our record-keeping obligations under pharmacy and health legislation.
Who we share it with
We disclose personal and health information only where it is necessary or required — for example to your prescriber or another treating health practitioner, to Services Australia and other government agencies for claiming, to My Health Record where you have not opted out, to the software and dispensing providers who host our systems under contract, and where we are compelled by law.
We do not sell personal information, and we do not disclose it overseas except where a service provider stores data outside Australia under contractual privacy protections.
This website
This site does not collect personal information directly. It does not run advertising or tracking cookies. Two things on the site are provided by third parties and are covered by their own privacy policies:
- Online bookings at bookings.mediadvice.com.au, operated for the MediADVICE network.
- The online shop at mediadvice.com.au, operated by the MediADVICE network.
- The embedded map on our contact page, provided by Google.
Our web host records standard server logs, including IP addresses and pages requested, for security and diagnostics.
Keeping it secure
Health records are kept in secure dispensing systems with access limited to pharmacy staff who need it. Paper records are stored securely on the premises. We retain records for the period required by law and destroy or de-identify them when that period ends.
Access, correction and complaints
You can ask to see the personal information we hold about you, and ask us to correct it if it’s wrong. Speak to the pharmacist in charge, call (02) 9631 3177, or email [email protected]. We’ll respond within a reasonable time.
If you’re not satisfied with how we’ve handled your information or your complaint, you can contact the Office of the Australian Information Commissioner at oaic.gov.au.
